Fast forward a bit—after gaining power from Orochimaru, the first thing Sasuke did was seek revenge against Itachi. He felt stronger than ever, and unlike his past self, who was full of arrogance, this time he genuinely believed he had the strength to kill Itachi—though there was still a sliver of uncertainty in his heart.
And it wasn't without reason that Sasuke felt uneasy. Just think about all the times he'd taken a hit to his pride. Every time he gained confidence, someone would show up and give him a harsh reality check.
When he first wanted to flex with Chidori, he ran into Haku. Even after learning from it and matching Naruto's level, he still got absolutely wrecked by Itachi later on. From a young age, he'd faced countless setbacks, which also forced him to grow up fast.

Tracking Itachi wasn't hard. Somehow, Sasuke could sense the chakra of people around him, though he didn't know exactly how it worked. It was just... instinct.
When he arrived, Itachi looked up with a complicated expression. His gaze softened as he saw Sasuke in the sky.
"Sasuke... you've grown. Even without me, you can survive in this cruel ninja world now..."
But then he shut his eyes, and when he opened them again—they were cold.
Itachi had decided. He would play the villain to the bitter end. Sasuke didn't need to know the truth. All he had to do was survive.
"Oh? My foolish little brother, here to challenge me again? This time, your eyes belong to me! Soon, I will gain the power of the Eternal Mangekyō Sharingan!" Itachi laughed maniacally, a tone completely unlike his usual self. A black flame suddenly burst from his eyes—Amaterasu (Heavenly Illumination).
Wrapped in lightning, Sasuke sneered. He raised a single hand to the sky—and a thunderbolt as thick as a tree trunk slammed down. The black flames, supposedly unquenchable, were wiped out by the lightning.
Blood dripped from Itachi's eyes. He summoned Susanoo, the massive red exoskeleton forming around him. Black flames surged again—this time coating his blade.
The crimson specter held a shield in one hand and a long blade in the other. He swung.
Sasuke, unfamiliar with his new power, panicked and transformed into lightning to dodge. But Itachi's Susanoo sword had a suction effect—he was pulled in, even in lightning form.
If he hadn't reacted in time and become lightning, he would've been diced into pieces. But even in elemental form, he couldn't fully escape the blow.
When Sasuke reformed, his clothes were burnt and clinging with the cursed black flames. He quickly threw them off. The moment they hit the ground, they turned to ash.
Sasuke steadied his mind. He realized—he had to be careful.
He retaliated with lightning that pierced through the sky like a spear, moving at the speed of light. Itachi couldn't dodge. A hole tore through his chest. He fell.
At least... that's what Sasuke thought.
A sharp pain suddenly stabbed at Sasuke's eyes.
Before he could even enjoy the victory, he was yanked back to reality.
Rubbing his eyes, he realized—it had all been an illusion. Itachi hadn't fallen at all. He stood right in front of Sasuke, reaching for his forehead.
Sweat poured down Sasuke's face. He didn't even know when he'd been trapped in Tsukuyomi (Moon Reader).
The air cracked with electricity. Lightning surged as Sasuke's rage boiled over.
"Die!" he shouted, thunder erupting around them. His lightning stripped away the outer layers of Itachi's Susanoo, eating away at it like termites.
Blood spilled from Itachi's mouth as he kept walking forward. Though Sasuke had the advantage, he backed away until his heel hit stone.
Itachi kept approaching—one slow, painful step at a time. Blood pooled under his feet, marking every step with red.
His Susanoo was fading, nearly transparent—but still, he walked on.
Sasuke increased the voltage, but nothing stopped Itachi.
Itachi's hand reached out, inches from Sasuke's eyes. Sasuke was frozen. His body wouldn't move, as though shackled by something unseen. All he could do was watch Itachi's fingers get closer.
He was drenched in sweat, his Sharingan fading. The lightning around him finally died out. Only now did he realize—this lightning was nearly impossible to maintain.
Breathing hard, Sasuke stood there. Itachi's hand finally landed gently on his forehead. The thunder dispersed.
Itachi's lips moved. He said something... but Sasuke couldn't hear anymore.
He only remembered one thing—Itachi fell before him, smiling.
If he had wanted to, he could've taken Sasuke with him in death. But he didn't.
Uchiha Itachi was dead.
